Rejuvi Health is a natural health advisory platform doing business as Rejuvi Health ("Rejuvi Health," "we," "us," or "our"). Our platform provides AI-assisted health intake and personalized supplement protocol recommendations through our website at rejuvi.health ("Platform").

The information, protocols, and supplement recommendations provided through Rejuvi Health are for educational and informational purposes only. They are not intended to:

To use Rejuvi Health, you must:

By creating an account, you represent and warrant that you meet these requirements.

A. Account creation. You must create an account to access the Rejuvi Health intake and protocol features. You may use your email and password, or sign in with Google.

B. Account security. You are responsible for maintaining the confidentiality of your account credentials and for all activity that occurs under your account. Notify us immediately via the contact form if you believe your account has been compromised.

C. Accurate information. You agree to provide accurate, current, and complete information during registration and to update your information as necessary.

D. One account per user. You may not create more than one account. Duplicate accounts may be terminated.

You agree to use Rejuvi Health only for lawful purposes. You agree NOT to:

A. Nature of recommendations. The supplement protocols generated by Rejuvi Health are personalized recommendations based on the information you provide during the intake process, produced by an AI system trained on functional and integrative health principles.

B. Not medical advice. Our recommendations are not medical advice and should not be treated as such. They are educational recommendations for nutritional supplementation.

C. Accuracy of your information. The quality of your protocol depends entirely on the accuracy of the information you provide. You are solely responsible for the accuracy of what you submit.

D. Medication interactions. While Rejuvi Health makes reasonable efforts to flag potential interactions between recommended supplements and the medications you disclose, we cannot guarantee that all interactions have been identified. Always consult your physician or pharmacist before adding any supplement to your regimen if you take prescription medications.

E. Athlete compliance. For NCAA and competitive athletes, Rejuvi Health recommends only NSF Certified for Sport® products. However, it is your responsibility as an athlete to verify the compliance of any supplement before use. Rejuvi Health is not responsible for changes in a product's certification status after the date of your protocol.

Supplement orders are fulfilled through Fullscript, a third-party professional supplement dispensary platform. When you click to order your protocol, you will be redirected to Fullscript's website. Your use of Fullscript is governed by Fullscript's own Terms of Service and Privacy Policy, available at fullscript.com/legal/terms-of-service.

Rejuvi Health is not responsible for:

Rejuvi Health does not process supplement orders and does not handle payment card information for supplement purchases.

A. Affiliate eligibility. Participation in the affiliate program is subject to a separate affiliate agreement.

B. NCAA compliance. For student-athletes participating in our NIL affiliate program, it is your responsibility to ensure that your participation complies with your institution's NIL policies and applicable NCAA rules. Rejuvi Health is not responsible for any compliance issues arising from your participation in our affiliate program.

C. Cookie tracking. When a user clicks an affiliate link, a 30-day tracking cookie is set. Affiliate commissions are credited based on this tracking period.

All content on the Rejuvi Health platform — including text, graphics, logos, and software — is the property of Rejuvi Health and is protected by applicable intellectual property laws. You may not reproduce, distribute, modify, or create derivative works from any content on our platform without our express written permission.

Your use of Rejuvi Health is also governed by our Privacy Policy, available at rejuvi.health/privacy, which is incorporated into these Terms by reference.

REJUVI HEALTH IS PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED. WE DO NOT WARRANT THAT:

TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, REJUVI HEALTH AND ITS OFFICERS, DIRECTORS, EMPLOYEES, AND AGENTS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF THE PLATFORM OR ANY PROTOCOL RECOMMENDATIONS. IN NO EVENT SHALL OUR TOTAL LIABILITY TO YOU EXCEED THE AMOUNT YOU HAVE PAID TO REJUVI HEALTH IN THE TWELVE (12) MONTHS PRECEDING THE CLAIM.

You agree to indemnify and hold harmless Rejuvi Health and its officers, directors, employees, and agents from any claims, damages, or expenses (including reasonable attorneys' fees) arising from:

We reserve the right to suspend or terminate your account at any time, with or without notice, for violation of these Terms or for any other reason at our sole discretion.

You may terminate your account at any time by contacting us using the form below.

These Terms are governed by the laws of the Commonwealth of Pennsylvania, without regard to conflict of law principles. Any disputes arising from these Terms shall be subject to the exclusive jurisdiction of the courts located in Westmoreland County, Pennsylvania.

We may update these Terms from time to time. We will notify you of material changes by email or by posting a notice on the platform. Your continued use of Rejuvi Health after any changes constitutes your acceptance of the updated Terms.
